Prescription Advantage Program is a State program that will assist you with copayments for your Medicare Part D Plan, if you reach the Medicare coverage gap or donut hole. You are also entitled to a Special Enrollment to join a Part D plan if you do not have one or change plans once a year.

MCPHS Pharmacy Outreach Program
MCPHS – Mass College of Pharmacy and Health Sciences Pharmacy Outreach Program- This is a free prescription drug and information referral service for Massachusetts and residents. The Pharmacy Outreach Program offers medication counseling, prescription drug plan referrals and other prescription drug assistance such as information on manufacture assistance that may be available.
MCPHS Patient Clinic and Treatment Centers-The following clinics are also available at the MCPHS campus in Worcester: Eye and Vision Clinic, Forsyth Dental Hygiene Clinic, The Balance Movement and Wellness Center and the NESA Acupuncture Treatment Center. Massachusetts Council on Aging (MCOA) is a non-profit membership organization comprised of municipal Councils on Aging. Their mission is to support the independence of adults 60 and older by advocating for programs and services to meet their needs, promote the growth and quality of Councils on Aging and senior centers and strengthen the professional skills of Council on Aging staff.

Medicare is the federal health insurance program for people who are 65 or older, certain younger people with disabilities, and people with End-Stage Renal Disease (permanent kidney failure requiring dialysis or a transplant, sometimes called ESRD).

One Care is a new health care option for Massachusetts adults, age 21-64, who are eligible for both MassHealth and Medicare (dual eligible). The One Care target population includes non-elderly adults with physical disabilities, developmental disabilities, serious mental illness, and substance abuse disorders. The goal of One Care is to provide higher quality care for dual eligible adults under age 65 at a lower cost than MassHealth and Medicare provide separately.

Aging Services Access Points (ASAPS) are one-stop entry points for all of the services and benefits available to seniors in Massachusetts. These agencies provide information, applications, direct services, and referrals. ASAPs manage the state-based Massachusetts Home Care Program.
